Your Appointment - We see patients by appointment, however we will always be happy to see a patient without a scheduled appointment at any time.
We all do our best to see that you are seen at your scheduled appointment time. There are times we may ask for your patience and understanding when we fall behind due to emergencies that arise and patients who may require more time than anticipated.
Your time spent in our office may be due to the nature of your visit and testing performed. Every patient seen in our office will be given individualized attention.
Prescription Refills - Prescriptions may only be refilled during business hours for record keeping purposes. We would also advise that you call at least three to five days before you run out of medication completely. The pharmacy will be called within 24 hours of your call to our office.
You will also be asked to bring all current medications with you to your appointments. This is for your protection to keep your physician aware of all changes that may have been made to your prescription regimen and they can adjust your medication accordingly.
Your Medical Record - Your medical record is completely confidential. We will not release any information regarding your condition without your written consent. The only exception to this would be a court order to provide information.
About Your Bill - We would appreciate payment for your medical care at the time services are rendered. For your convenience, we accept cash, checks, MasterCard or VISA.
If your insurance carrier is an HMO, you are required by your insurance company to bring a valid referral form, completed by your Primary Care Physician, at the time of your visit. As you know, we cannot see you without this completed referral form.
